No, once you are eligible for Medicare and have been enrolled by our office in the plan’s Medicare Supplement plan, you should not show your PTSMN Card. You will receive a card from Medicare and one from the Medicare Supplement Plan. If your spouse is not yet age 65, he or she will still use the PTSMN card until eligible for Medicare. NOTE*** Retired members and their spouses are contacted by the PTSMN office 6 weeks before they turn 65 so that they can be enrolled in the plan’s Medicare Supplemental plan. Retired members and their spouses should contact the PTSMN office immediately if they become eligible for Medicare due to disability prior to age 65.

Both active and pre-medicare retiree members should present the PTSMN card with the LaborValue Rx information to the pharmacy when making a prescription purchase. This plan provides prescription drug discounts and has a 20% co-pay* at the time of purchase. The balance is billed directly to PTSMN and the deductible is waived. This is a white card with blue printing of the PTSMN logo on the top left and the LaborValue Rx logo under that with the prescription drug information is in red printing. The eligible family members are printed on the back.

As of 8/1/17, the PTSMN cards include the Delta Dental ID information for Active members. The green & white Delta Dental logo is on the left side of the front of the card. The eligible family members are printed on the back. Retirees who elect to pay for the dental benefits that are offered from PTSMN will receive a separate ID card from Delta Dental to show at the dentist office.

No, you are free to see any provider. To access the highest level of benefits we utilize HealthPartners Open Access as our Preferred Provider Organization (PPO). For a list of these providers you can go to www.healthpartners.com/openaccess. For physician office visits, in-network charges are paid at 100% after a $25.00 co-pay, out-of-network is paid at 80% of usual and customary charges after the annual deductible has been satisfied. In the PTSMN plan no referrals are required. You can see the providers or specialists as you see fit. For Mental Health and Substance abuse care, members may HealthPartners EAP, our employee assistance program.

Dependent children are covered through the month in which they turn 26.
